Completing this form does not guarantee Furry Friends Network will be able to place this stray cat/kittens in our foster program or give assistance in helping to rehome this animal. This form will allow Furry Friends Network to assess this pet’s background, personality and needs. A time will be scheduled for us to meet the animal should the review of this form determine that is necessary.

• Most vets and/or shelters will scan stray animals for free to be sure they are not microchipped and have an owner awaiting their return. We ask that you contact a local vet/shelter to have the animal scanned before completing the form below.

• FFN requires a minimum $25 donation to take a cat/kittens into our program. Additionally, we require the cat/kittens be tested for FeLV/FIV/HW (and be negative) prior to entering our program. FFN is not able to accept any cat/kittens that are FeLV or FIV positive.

Once you have submitted this form you will need to send a few digital pictures of this animal (standing, sitting, etc.). Please make sure the pictures are no larger then 200k in file size. Email them to FurryFriendsInfo@verizon.net with the following subject line (Your Name and the words STRAY/FERAL QUESTIONNAIRE).

Please note Furry Friends Network will consider stray/feral cats & kittens only from the Central Pennsylvania area.
